What are Form Templates?

Form templates are pre-configured digital documents and are used in various modules, including:

  • Incidents: form templates enable responders to relay relevant information from the field to the dispatcher and vice versa. You can link one or more form templates to each incident type. These forms appear in the Dispatcher and in the mobile application when an incident is initiated.
  • Assets: form templates provide the dispatcher and mobile application with additional information about an asset. Each asset can have only one form linked to it. The asset form is accessible and can be edited by any user with the appropriate permissions (Dispatcher or mobile application user).
  • User updates: form templates enable users to share updates that are relevant to their status or resource needs. You can attach one or more form templates as an in-incident user update or as part of a policy to determine a user’s transition from one policy to another.

Creating Form Templates

To create a new form in the system, you must have the appropriate permissions to access system settings.

To open the Form editor:

  1. Click on Settings (gear) in the upper left corner of the Dispatcher screen.

  1. Click on the Incidents tab and then select Form editor.

  1. In the Forms table, you can:
  • Create an entirely new form from scratch by clicking on the + symbol, or
  • Duplicate an existing form and edit the content by selecting the form, right-clicking and selecting the Duplicate option.

Creating a New Form Template

You can create a new template completely from scratch.

To create a new form:

  1. In the Forms table, select +. The New form window opens.

  1. Add a name in the Form name field.
  2. Assign it a Form type.
  3. If you have selected Form type Incidents, then the Assign to option appears in the dropdown list and you can select all incident types to which this form will be assigned.
  4. Click on Create form.

Populating the Form

After creating a new template, you must populate it with the required fields. The following fields are available:

  • Text box: a free text space limited to a certain length.
  • Text area: free text space where you can enter a description of the incident/assets (limited in size according to your defined settings).
  • Title section: form/section header.
  • Radio buttons (+ panel): Create a list of options from which the user can select one or multiple options.
  • Checkbox: Allows the user to select an option.
  • Image upload: Allows the user to capture and send an image.
  • Date time picker: enables the user to enter a date and time.
  • Location pin: the latitude and longitude of the user/incident.
  • Timestamp: the time when the form was completed.
  • Signature: Allows the user to insert a signature into the form.
  • External video: Allows the user to capture and send a video.
  • Panel: The panel to which the radio buttons are added.
  • Dropdown select: Allows the user to choose from a dropdown list.
  • Barcode scan: Allows the user to scan a QR code and compare that scanned code with a predefined list.

Step 1: Open the Form Template

To begin, open the form template you want to populate. This template contains a number of fields that you can fill in.

Step 2: Fill in the Fields

To fill in the fields, follow these steps:

  1. Drag the required fields from the left panel to the left column.Dragging fields

  3. The basic settings allow you to configure the field with a name, number of selectable options. Where possible, multi-select and auto-complete can be added to the field.b. Open the Requirements tab and set the visibility of the field. Requirements
    Open the Interfaces tab and set the field names for interfaces such as API integrations that your organization may have, as well as the PDF field name for the form.

  4. Repeat the process until you have all the fields within your form.
  5. Click Save.

Step 3: Publish the Form

Now you are ready to publish the form so that others can fill it in. You can publish the form in a number of ways, such as by sharing it via a link, integrating it on your website, or embedding it in an email.
